About the Role

We're looking for a Care Manager who's dedicated and compassionate. They'll assess patients, create care plans, and coordinate services to improve healthcare outcomes.

Care Manager Responsibilities and Duties

• Conduct comprehensive assessments of patients' physical, emotional, and social needs.

Education and Experience Requirements

• Bachelor’s degree in Nursing, Social Work, or a related field.
• Current licensure as a Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or equivalent as required by state regulations.
• Minimum of 2 years of experience in care management or a related healthcare setting.
• Certification in Case Management (CCM) is preferred.
